Sua etiologia é multifatorial, consiste em um estreitamento da arcada superior, apresentando palato ogival profundo, muitas vezes associado à disfunção respiratória. Os supranumerários posicionados entre os incisivos na maxila, na região da linha média, são chamados de mesiodens. Eles raramente irrompem espontaneamente e, em grande parte, encontram-se invertidos. No diagnóstico de mesiodens, irrompidos ou não, o planejamento do tratamento ortodôntico deve objetivar a redução dos comprometimentos estéticos e funcionais. O tratamento consistiu em colagem direta de acessórios ortodônticos de prescrição Roth com slot .022”x .030”, molas abertas entre os dentes 33 e 31, desgastes interproximais dos incisivos inferiores para dissipar o apinhamento, alinhamento, nivelamento, uso de elástico corrente na arcada inferior, elásticos de Classe III 3/16” médio do lado direito e elásticos de intercuspidação 1/8” médio bilaterais. Após a correção da má oclusão, foi feita remoção dos acessórios, seguida de instalação de contenções do tipo placa de Hawley no arco superior e fixa 3X3 no arco inferior. O protocolo eleito para o uso da contenção superior foi 24 horas por dia durante 6 meses, seguido de uso noturno por mais 6 meses e uso indeterminado da fixa inferior.
