O resultado favorável do retratamento endodôntico depende da remoção eficiente do material de preenchimento e bactérias residuais do sistema de canais radiculares. Diante de situações como essa, o cirurgião dentista precisa avaliar o caso para saber das possibilidades de se remover ou não esse fragmento. Existem várias opções terapêuticas que podem ser realizadas, como por exemplo: remoção do fragmento via canal; passar pelo fragmento ou não, sem conseguir removê-lo, e obturar logo em seguida; cirurgia parendodôntica. No caso clínico em questão, com o auxílio de uma tomografia, foi tomada a decisão de não remover o fragmento, devido a atresia do canal e sua dupla curvatura, e tentar ultrapassa-lo com uma lima de menor calibre, realizando uma obturação envolvendo o fragmento da lima com a massa obturadora. Finalizado o tratamento, foi feito uma proservação radiográfica depois de 4 meses e após um ano foi realizada uma nova tomografia, ambos mostrando sinais sugestivos da redução da lesão periapical existente antes do tratamento, sugerindo assim sucesso do caso. A paciente se recusava a realizar extração dentária, por isso foi submetida a tratamento ortodôntico com aparelho fixo sem extração. Mini implantes foram utilizados para promoverem a distalização dos dentes do lado esquerdo que estavam em Classe II. Apenas um mini-implante extra alveolar na região do buccal shelf e aparelho convencional Morelli Padrão III Capelozza reverteu a má oclusão retraindo e girando distalmente todo o arco inferior para o lado direito. O momento de força criado entre o mini-implante e o gancho de retração em relação ao centro de resistência da arcada inferior, determina a quantidade de retração, a intrusão dos molares e extrusão dos incisivos. Os mini-implantes podem proporcionar ancoragem extra-alveolar ou inter-radicular. Três posições para os mini-implantes são propostos para retrair e girar a arcada inferior: 1. Buccal Shelf da mandíbula, 2. Distal à raiz do primeiro molar inferior e 3. Na crista infrazigomática. Todas as três posições efetivamente retraem e giram arco inferior, mas os mini-implantes inter-radiculares interferem no trajeto do movimento dentário, e os parafusos na crista infrazigomática não intruem os molares inferiores. Portanto, as conclusões são: 1. Os miniparafusos extra-alveolares no buccal shelf mandibular produzem uma linha ótima de força para tratamento conservador da Classe III com mordida aberta.
