Sendo muito frequente na prática ortodôntica e não apresentando prevalência entre os gêneros masculino e feminino. Não apresenta um bom prognóstico devido a sua alta taxa de recidiva. Tem etiologia multifatorial e precisa de um diagnóstico diferencial, elaborado e específico. O presente relato de caso clínico apresenta uma alternativa para o tratamento da sobremordida profunda, associada à colagem diferenciada dos acessórios anteriores dos incisivos superiores e inferiores, onde foi reduzido o valor padrão (-0,5 mm para incisal do centro da coroa dos mesmos) logo no início da mecânica. Foram colocados batentes posteriores, o que facilitou a colagem do arco inferior; posteriormente foi realizada a acentuação e reversão da curva de Spee, o que auxiliou na abertura da mordida, diminuindo a sobremordida anterior.
