Além disso, a etiologia da má oclusão de Classe II pode estar associada com a impacção de caninos permanentes, provavelmente por causa do arco superior ser mais estreito, contribuindo para a impacção por vestibular. Este artigo objetivou relatar um caso clínico de uma paciente com má oclusão de Classe II, apinhamento superior, com vestibuloversão do canino superior direito e impacção do canino superior esquerdo por meio da extração atípica do canino impactado. Optou-se pela extração do dente 14 e 23, mantendo o dente 24, já que este se encontrava em boa oclusão e, uma vez que o canino superior esquerdo estava em uma posição desfavorável e seu tracionamento seria duvidoso com risco de reabsorção radicular do dente 22 devido a sua proximidade com a raiz do mesmo. A extração do canino superior permitiu a correção da má oclusão de Classe II e apinhamento superior, com manutenção do bom perfil facial e estabilidade a longo prazo. Além disso a perda de dentes permanentes geralmente causa a extrusão do elemento do arco oposto, e proclinação, rotação, ou inclinação dos dentes adjacentes, alterando todo o plano oclusal. Este artigo objetivou relatar o tratamento de uma paciente com má oclusão de Classe II, apinhamento superior e inferior, com transpasse horizontal acentuado, com alteração do plano oclusal. Os traçados e as medidas cefalométricas demonstram a retrusão maxilar e dos incisivos superiores e molar superior, verticalização dos molares inferiores, e melhora do perfil tegumentar. Este caso clínico evidenciou a possibilidade de reabilitação oclusal em uma paciente com má oclusão de Classe II com perda de dentes inferiores por meio de implante osseointegrado inferior. Entretanto, para se obter este resultado foi necessário a utilização de miniimplante durante o tratamento ortodôntico para intrusão do molar superior e retração. O miniimplante foi uma ancoragem extremamente importante para a reabilitação da oclusão do paciente.
