No entanto, neste tipo de pacientes existem alterações típicas que devem ser consideradas, como a presença de proclinações, diastemas, rotações, erupções excessivas, bem como escassez do osso de suporte. Além disso, para que um procedimento ortodôntico seja bem-sucedido, o fator infeccioso a nível periodontal deve ser controlado, deve haver suporte ósseo suficiente, apresentar dois terços do comprimento da raiz, bom estado de saúde geral e contar com um paciente cooperativo em todas as fases. Pensando nisso, foi desenvolvida essa pesquisa, cujo principal objetivo foi identificar os aspectos centrais a serem levados em consideração, durante o tratamento ortodôntico, quando se trabalha com pacientes que apresentam lesões periodontais. Para isso, foram revisados um total de 36 documentos publicados em revistas científicas contidas em bases de dados como PUBMED, LILACS, SCIELO, nos quais ao final se verificou que qualquer que seja a condição periodontal encontrada, o tratamento pode ser efetuado, no entanto, os movimentos devem ser mais lentos e menos intensos que quando se trabalha com pacientes periodontalmente saudáveis, ou seja, permanecer nos mínimos terapêuticos. Inicialmente realizou-se o isolamento absoluto do campo operatório. Por isso, quando houver prognóstico desfavorável daquele elemento dentário, existe a necessidade de realizar uma reintervenção endodôntica. Mas, para que isso aconteça, é necessário que sejam garantidos princípios científicos, biológicos e mecânicos capazes de promover a limpeza e modelagem do canal radicular. Este trabalho tem como objetivo relatar um caso clínico de retratamento do elemento dentário 26 com sintomatologia persistente. As limas de escolha para a desobturação foram: LOGIC RT #30.10, #25.08 e 20.06. Assim que todos os canais foram desobturados, lançou-se mão da instrumentação com as limas RECIPROC BLUE (VDW) #25.08, SEQUENCE ROTARY FILE ( MK LIFE) 35.04 e X1 BLUE (MK LIFE) #40.06. Utilizou-se como solução irrigadora o Hipoclorito de Sódio 2,5% e, já que havia persistência de sintomatologia, optou-se por utilizar protocolo de irrigação e agitação com EDTA e EASYCLEAN, a fim de garantir que o elemento dentário estava desinfectado o suficiente para ser obturado. Pode-se concluir que para que exista uma chance maior de prognóstico favorável de uma reintervenção endodôntica, é importante a utilização de ferramentas para melhor irrigação e desinfecção dos canais radiculares, juntamente com um bom selamento radicular e coronário. Além disto, é necessário que haja proservação do elemento dentário por pelo menos 1 ano através do exame clínico e radiográfico, e, ao longo dos anos o paciente continue sendo acompanhado.
