A mordida aberta, caracteriza-se pelo trespasse vertical negativo que ocorre na região anterior, quando os molares estão em oclusão. Sendo assim, há uma necessidade de abordagem cada vez mais precoce em relação ao tratamento das oclusopatias. Dentre as várias maloclusões, a mordida cruzada anterior (MCA) e a posterior (MCP) devem receber especial atenção, pois afetam diretamente o crescimento maxilomandibular diretamente. O objetivo deste trabalho é apresentar uma nova abordagem para o tratamento da MCA e da MCP, através do caso clínico de um paciente de 4 anos e 3 meses, com agenesia de incisivos laterais decíduos e permanentes, MCA e MCP unilateral funcional. Foi utilizado um disjuntor com recobrimento oclusal e de palato (disjuntor modificado) associado a uma placa inferior de resina (HGS modificado) com elásticos de Classe III e, como contenção, um aparelho funcional para estabilizar a função. O tratamento teve uma fase ativa de aproximadamente 7 meses e uma fase de contenção de aproximadamente 6 meses, todo ele com aparelhos intrabucais, confortáveis e estéticos, sem qualquer elemento externo.
