Na impossibilidade do diagnóstico precoce, esforços devem ser empregados para reposicionar o dente no arco dentário evitando sua extração, dada a sua importância no equilíbrio, na harmonia e na função do arco dentário. Apesar de o canino superior representar o dente que mais apresenta anomalias de irrupção, depois dos terceiros molares, a prevalência da retenção de caninos, na população, é baixa. Desempenham um importante papel no estabelecimento e manutenção da forma e função da dentição, sendo sua presença no arco dentário essencial para o estabelecimento de uma oclusão dinâmica balanceada, além da estética e harmonia facial. Assim, dada a sua importância no arco dentário, diante de uma impactação do superior permanente, esforços deverão ser empregados para manter o dente evitando sua extração. Sua etiologia multifatorial envolve fatores gerais e locais. Atribui-se grande importância à realização do diagnóstico precoce, na tentativa de prevenir a retenção do canino superior com trajeto ectópico de irrupção, interagindo, para este fim, aspectos clínicos e radiográficos. Existem várias condutas para o tratamento da retenção, selecionadas segundo o posicionamento dos caninos: a presença de alterações patológicas locais, as características da má oclusão e a idade e a disponibilidade do paciente.
