Sendo muito frequente na prática ortodôntica e não apresentando prevalência entre os gêneros masculino e feminino. Não apresenta um bom prognóstico devido a sua alta taxa de recidiva. O paciente foi tratado utilizando aparelho fixo superior e inferior, mini-implante, elástico de Classe II do lado direito para fechamento do espaço do dente 46. A mesialização do molar com a utilização do mini-implante com cantilever, mostrou-se bastante eficiente, sendo que o mini-implante foi removido em 6 meses, após o dente 47 estar em uma posição satisfatória. Após o tratamento, o paciente apresentou uma boa relação dentária mostrando uma má oclusão de Classe I do lado esquerdo e direito. Não houve mudança significativa no perfil facial do paciente. Após 5 anos da finalização do caso, o tratamento de fechamento de espaço mostrou-se estável.
