A ocorrência desses traumas vem aumentando nos últimos tempos, tornando-se mais um agravo na saúde pública do Brasil. Os traumatismos orofaciais mais freqüentes são: avulsão e fratura dental, e a sua maior incidência é no incisivo central superior. Muitos desses acidentes ocorrem em crianças em atividades recreativas ou dentro de casa. A grande maioria dos pais, professores e cirurgiões dentistas não estão aptos a fornecer um atendimento eficiente. Diante de uma avulsão dental é necessário uma boa condução do caso, pois as chances de perda do dente que sofreu avulsão são grandes. A ausência dos dentes em região anterior por trauma provoca além da perda de função, comprometimento da estética, interferindo até na auto estima do paciente. O plano de tratamento decorrente da ausência dos incisivos centrais superiores deve se basear em um minucioso diagnóstico, considerando a possibilidade de fechamento ou manutenção do espaço, por meio da mecânica ortodôntica ou orto/protética em busca de estética dental e/ou funcional. O problema mais comum, encontrado em cerca de 9,4% da população e em quase 30% dos pacientes ortodônticos adultos, é a deficiência transversal que tem sua principal apresentação pela mordida cruzada posterior – podendo envolver um elemento dentário ou um grupo de dentes – e é também observado nas más oclusões de Classe II, como compensação ao relacionamento deficitário da maxila com a mandíbula mesmo quando o cruzamento dentário não está presente. Trabalhos apresentam alta taxas de incidência do problema na população e esses números, além de altos, preocupam devido às controvérsias relacionadas ao tratamento não cirúrgico nos adultos, à dificuldade de tratamento no período não ideal (maturação esquelética completa), à falta de estabilidade no período pós-tratamento e, também, pelo fato de que poucos pacientes apresentam disposição, e/ou condições financeiras, para se submeterem ao tratamento cirúrgico.
