Por si só, pode causar sérios comprometimentos estéticos. Estando atrelada a agenesias múltiplas, o impacto na autoestima do paciente pode ser ainda maior. Sendo assim, o objetivo desse trabalho é descrever um caso clinico de um paciente com má oclusão de classe III apresentando múltiplas agenesias, onde foi eleito o tratamento ortopédico com expansão rápida da maxila associado ao uso da máscara de protração maxilar, e em seguida, o tratamento ortodôntico convencional, realizando- se o fechamento dos espaços das agenesias com o auxílio de mini implantes. Evidenciou-se após o tratamento, melhora no posicionamento maxilar em relação à base do crânio, aumento no comprimento efetivo da maxila; melhora na relação maxilomandibular; vestibularização dos incisivos superiores e extrusão dos molares superiores, correção da mordida cruzada anterior e da sobremordida profunda, diminuição do perfil côncavo, deslocamento anteroposterior dos molares permanentes fechando os espaços das agenesias dos pré-molares. Os caninos decíduos foram reanatomizados com estética, e mantidos em posição até que haja a necessidade de reabilitação com implantes. Três anos após finalização do caso, o mesmo se encontra estável, sem recidivas.
