Nos indivíduos adultos com maturação óssea avançada, a correção da deficiência transversal da maxila torna-se um desafio para o ortodontista. O tratamento dessa má oclusão, cuja característica dentária predominante é a mordida cruzada posterior, é controverso e suscita muitas dúvidas, pois variadas são as possibilidades de tratamento com entrega satisfatória de resultado nos pacientes adultos. Por causa disto este trabalho tem como objetivo relatar os possíveis procedimentos ortodônticos para tratar a mordida cruzada posterior em pacientes que não apresentam mais o crescimento, ou seja, nos adultos. O que possibilitou compreender, que a mordida cruzada posterior quando tratada em adultos, tem como característica primordial a correção por meio da cirurgia ortognática, contudo, com o avançar das tecnologias novas possibilidades foram inseridas como tratamento desta condição. O presente trabalho apresentou uma revisão da literatura sobre o tratamento da mordida cruzada posterior e ainda relatou um caso clinico onde foi realizada a expansão rápida da maxila. O caso clínico relatado foi de um paciente do gênero masculino, 12 anos de idade, diagnosticado com mordida cruzada posterior (MCP) e atresia maxilar; para tal tratamento foi eleito o aparelho de expansão maxilar tipo Hyrax. O protocolo de ativação foi de ¼ de volta pela manhã e ¼ de volta à noite, até que se conseguisse a sobremordida desejada. Após o periodo de ativação o aparelho foi travado e mantido como contenção durante 7 meses. Verificou-se que o tratamento de expansão maxilar foi eficiente no tratamento da mordida cruzada posterior, e ainda melhorou a assimetria facial.
